Maintenance Request Form

Please contact our office in addition to submitting this form if there is a problem involving any of the following:
  • If you don't have hot or cold water
  • If you don't have heat or your heat is not working properly
  • If you have a hazardous electrical problem
  • If you are having a problem with your refrigerator, range/oven or any other major plumbing fixture supplied by the landlord.
For all other repair requests please complete and submit the following information. The Landlord Tenant Law states the required waiting time for repairs is:
  • 24 hours for no heat or cold water, heat or electricity, or for a condition imminently hazardous to life.
  • 72 hours for repair of refrigerator, range and oven, or a major plumbing fixture supplied by landlord.
  • 10 days for all other repairs.

Please provide as much detailed information as possible, including the location and complete description of the problem. Failure to provide a detailed description of the problem may result in a delay of the work being started.

Example:
WRONG "Leaky toilet"
RIGHT "My toilet in the upstairs bathroom has a slow leak around the base"

Example:
WRONG "Garbage Disposal broken"
RIGHT "I turned my garbage disposal on today and it ran for a few seconds and then stopped. I flipped the switch on and off a few times and it's only making a humming noise now"

Please submit an additional form if you have more requests.

   I do not authorize entry into my unit to perform the repair if I'm absent.*


Note: This work order request does not mean a repair will occur. If you do not see a repair or hear from this office regarding your request, you are welcome to call the office for explanation. Please refer to your lease and security deposit agreement for repairs that will be your financial responsibility.
